FDAP QUALIFICATIONS :: Drug & Alcohol Prof. Certification
Applications
Note - Please read all the documentation relating to this scheme
before submitting an application. We regret that payments accompanying
an unsuccessful application can not be refunded.
Initial certification
To submit an initial application for either Registration or Accreditation
applicants must supply:
- Application form (this form is for both Registration
& Accreditation) [pdf]
- Workplace
assessment - comprising individual unit reports for all units for which the applicant does not have direct, externally validated, evidence of competence plus Senior Manager Approval form
- Copies of any relevant qualification certificates - endorsed by the applicant's
line manager / supervisor
The cost of an application for initial certification is £75 (reductions are available for those applying through an affiliated agency).
Upgrade
To submit an application for upgrade from registration to accreditation
applicants should contact us at office@fdap.org.uk.
The standard fee for an upgrade is £25.
Recertification
To be re-certified, applicants must supply, in support of their
continuing competence:
- Application form (the same form as per an initial application)

- Workplace assessment - comprising individual unit reports for all units for which the applicant does not have direct, externally validated, evidence of competence plus Senior Manager Approval form
The cost of an application for re-certification is £75.
Resubmission
Applicants may resubmit an application for certification / recertification
following an earlier unsuccessful submission.
The cost of resubmission is £25.
